What is alpha-gal syndrome?

Alpha-gal syndrome (AGS) is an allergy that can develop after certain tick bites. It makes the body react to alpha-gal, a sugar molecule found in mammals — so the triggers are meat from mammals (beef, pork, lamb, goat, venison, and more) and, for some people, other mammal-derived products. It's often nicknamed "red meat allergy," but that label can mislead: pork counts too, even though it's sometimes marketed as "white meat." The short version

"Alpha-gal" is short for galactose-α-1,3-galactose, a sugar molecule that most mammals — such as cows and pigs — make in their bodies, but people and other primates do not. The lone star tick carries alpha-gal in its own saliva. When it bites, its saliva delivers that alpha-gal into your skin along with other substances that rev up the immune system. In some people, the immune system responds by making antibodies (IgE) against alpha-gal — in effect, learning to treat it as a threat. (A common point of confusion: you're not reacting to a dose of another animal's blood being injected into you — you're reacting to alpha-gal carried in the tick's own saliva. Where the tick itself gets that alpha-gal is still being worked out; researchers think the tick's own biology, its gut bacteria, and possibly earlier blood meals may all contribute.) Once someone is sensitized this way, eating mammal meat — or, for some, other mammal-derived products — can trigger an allergic reaction, typically 2–6 hours later. Repeated tick bites can strengthen the response.

What causes alpha-gal syndrome?

In the United States, AGS is primarily linked to the bite of the lone star tick, and less commonly to the blacklegged tick or western blacklegged tick. Not everyone who is bitten by these ticks develops AGS, and researchers are still working to understand why some people develop it and others don't.

One feature that makes AGS confusing is timing: it can take weeks to months after a tick bite for the allergy to appear. Because the tick bite and the first food reactions can be so far apart, many people don't connect the two on their own.

Most reported U.S. cases occur in southern, eastern, and central states — the areas where lone star ticks are most common. People who spend a lot of time outdoors, such as hunters, may have a higher chance of tick exposure.

How common is it?

More than 110,000 suspected cases of AGS were identified in the U.S. between 2010 and 2022, and the CDC estimates that as many as 450,000 people may be affected. Because AGS isn't a nationally notifiable condition and can be under-recognized, the true number is not known and is widely believed to be higher.

Recognition is rising quickly. A 2026 CDC analysis of blood-donor samples found alpha-gal antibody (IgE) present in about 24% of donors in the five states with the highest levels (Arkansas, Kentucky, Missouri, Tennessee, and Virginia). Antibodies alone are not the same as having the syndrome, but the data point to a condition that is more widespread — and more often diagnosed — than it used to be. Most reported cases are in adults, though people of all ages can develop AGS.

What are the symptoms?

Reactions vary a lot from person to person and can range from mild to severe or even life-threatening. They usually appear 2–6 hours after exposure — a notable delay compared with many other food allergies. Reported symptoms include hives or itchy rash, nausea or vomiting, stomach pain, heartburn, diarrhea, shortness of breath, a drop in blood pressure, swelling of the lips, throat, tongue, or eyelids, dizziness, and anaphylaxis (a severe, whole-body reaction).

In an emergency, act immediately. A severe allergic reaction (anaphylaxis) — such as trouble breathing, throat or facial swelling, fainting, or a rapid drop in blood pressure — is a medical emergency. Call 911 or seek emergency care right away, even if an epinephrine auto-injector or nasal spray has been used.

How is it diagnosed?

Diagnosis is made by a healthcare provider, typically using a combination of a physical examination, a detailed history (including tick exposure and the timing of reactions), and a blood test for the antibodies the body makes in response to alpha-gal. A provider may also suggest allergy skin testing. Diagnosis belongs with a clinician — an allergist is often the right specialist — because the antibody result has to be interpreted alongside your history and symptoms.

How is it managed?

There is no vaccine for AGS, and there's no medication that removes the allergy. Most clinicians recommend three broad strategies, tailored to the individual:

Some people who avoid further tick bites may, over time, tolerate alpha-gal-containing products again — but that is an individualized medical decision to make with a provider, not something to test on your own.
